Current Patient Resources

Whether you have a question about your care or want to request your medical records, we’re here to help.

Online Patient Portals

Pre-appointment forms are available in the Oberd patient portal.

The Follow My Health patient portal requires enrollment to access. If you are an established patient and have not yet enrolled, please contact our office for assistance.

This portal provides you with 24/7 access to:

  • Send a secure message to your care team
  • View your health information, such as visit summaries and more.

On mobile devices you will need to download the Follow My Health App from the App store.

Medical Record Requests

We ensure your medical information remains private and confidential, and require a completed and signed authorization form before we release your records.

Online Patient Record Request:

We now offer an online request tool for the following individuals to request patient records:

  • Patients requesting their own records
  • Patients requesting their own records be sent to someone else
  • Parents of minor patients requesting records
  • Caregivers acting on behalf of a patient (i.e. Power of Attorney)

The online tool verifies your identity by asking for a photo of your driver's license or Government issued identification, which can be taken via webcam or smartphone.  There is no additional charge to use this service.

If you prefer you also have the options to download and print an Authorization for Release of Information Form, or call our Health Information Department at 651-312-1553 to request this form.

If you Choose not to use the online tool, please Return Your Completed & Signed Release Form by Mail or in Person to

FAQs About Obtaining Copies of Medical Records

Late Arrival Policy

New patients must complete the necessary paperwork prior to their first visit using the online patient portal. If you choose to complete the necessary paperwork in the office, you must arrive 20 minutes early. If the paperwork delays your appointment start time, your appointment might be canceled at the discretion of the provider.

If necessary, it is acceptable to print the Release of Information form and fill it out prior to arriving at the office.

If a patient arrives past their scheduled time and/or do not have their paperwork completed, their appointment might be canceled at the discretion of the provider.  This is to ensure that the patients who arrive on time do not experience delayed waits to see their provider. If you arrive late, you may be asked to reschedule. If you choose to wait, we will try to minimize your wait time as best as possible, however we need to provide timely care to our other patients as well.
